You may send us a written request at osa_career@hksyu.edu of amending your data. 本人同意及向秘書處披露參加者於參加表填上的個人資料以供政府用以進行意見調查，評估研究、舉辦培訓／分享會，執行資助計劃日常工作、監察獲資助團體的表現、符合法例訂明有關披露資料的規定、以及作其他與上述任何事宜有關的用途。此外，政府保留權利向第三方披露申請團體提供的資料，以供評審申請、完善本計劃等。

I hereby consent to the disclosure of the personal data I have provided in the application form to the Secretariat, for use by the Government in conducting opinion surveys, evaluation studies, organizing training or sharing sessions, carrying out the daily operations of the funding scheme, monitoring the performance of subvented organizations, complying with relevant legal disclosure requirements, and for other purposes related to any of the above. In addition, the Government reserves the right to disclose the information provided by the funded organizations to third parties for purposes such as reviewing the application and improving the scheme. 本人同意樹仁大學採用其名字、相片、錄像及相關資料作推廣和宣傳之用。
