重要事項 Important Notes:

1. 請於提交本申請表至少三個工作天後到宿舍管理部（研究院綜合大樓RLG403）領取閣下的住址證明。
Please proceed to HMU (RLG403, Research Complex) for the Address Proof/Previous Accommodation Certificate after three working days of the submission of this application form.

2. 申請住址證明/過往住宿證明收費為每份港幣$50，並將以八達通收取所需費用。
Each copy of Address Proof/Previous Accommodation Certificate costs HK$50 and shall be settled by Octopus payment.

3. 首次到香港升學的非本地新生均需要申請香港身份證和香港銀行戶口。因此，非本地宿生若於一年級第一學期申請住址證明，首兩張費用全免。
In order to facilitate application for bank account and Hong Kong ID card, each non-local undergraduate freshman staying in the dormitory is eligible to obtain two copies of address proof forfree in their first semester with Shue Yan.
