1. 此維修申請表僅供職員使用。
This form is for the exclusive use of staff to make maintenance request.

2. 在接獲申請後，本處一般會於七個工作天內，向申請人發送確認郵件及處理申請。
Upon receipt of the maintenance request, the FMO will confirm the applicant by email and handle the application within 7 working days.

3. 維修時間視乎所需工作的複雜程度。
The completion time is subject to the complexity of the maintenance work.

4. 如需緊急維修，請致電本處熱線 2804 8566。
For emergency maintenance request, please call the FMO hotline at 2804 8566.
