3. Please prepare the following supporting documents for upload in order to complete your application form:
- A recent, color, passport-size photo (i.e., approximately 40mm (Width) x 50 mm (Height) and show full frontal face with clear facial features.)
- A copy of your HKID Card or other identification document
- Photocopies of degree certificates
- A photocopy of each professional qualification obtained as stated on the application form
4. Applicants are required to pay an application fee of HK$200 by Credit Card (Visa or Master card) via the online payment system (a link will be sent to your email once the application form is submitted) Fees paid are not refundable.
5. 2 signed and sealed Referee Reports should be

6. Official transcripts of your undergraduate and postgraduate studies should be

sent to the Department of Counselling and Psychology in a sealed envelope from the issuing universities. (Download:

) Graduates of HKSYU are not required to request a transcript from the Registry. Copies of past results will suffice.
7. If you are a graduate of the current academic year, please provide the course information (course code and title) to be completed which has not been printed out in your submitted transcript on a separate sheet
8. For top-up degree holders, transcripts of sub-degree programme should also be provided.
9. An official score report of IELTS or TOEFL on English proficiency should be
directly
sent by examination bodies.
10. To study in Hong Kong, all non-local applicants are required to obtain a student visa issued by the Immigration Department of the HKSAR Government, except for those admitted to Hong Kong as dependants, who do not need prior approval before taking up full-time and part-time studies. Non-local applicants issued with a valid employment visa also do not need prior approval to pursue part-time studies. It is the responsibility of individual applicants to make appropriate visa arrangement.
11. Incomplete application will

be considered.
12. Shortlisted applicants will be invited to an interview by email.
13. If your application is unsuccessful, the form and all the supporting documents will be destroyed 8 weeks after the application deadline.
